| The Home & Community Care Program is jointly funded by the Commonwealth & State governments under the auspices of the HACC Act 1985. It provides funding for services which support people who live at home and whose capacity for independent living is at risk or who are at risk of premature or immature admission to long term residential care.
The program assists people in the target group who need basic maintenance and support services to continue living in the community with dignity.
